针药加微波治疗声带息肉及小结76例临床研究

摘　　要：目的观察针刺、中药和微波热凝相结合治疗声带息肉及小结的疗效.方法利用纤维喉镜引导软导线针状电极达到病变部位,并刺入病变组织1～2mm,广基底及较大息肉可分多点进行.选择功率40～50W,时间2～3秒,将病变组织凝固、气化.术后给予庆大毒素8万U、地塞米松10mg、α-糜蛋白酶4000U超声雾化吸入3天,每日2次,3天后改'亮嗓汤'雾化吸入,每日2次,每次20分钟.同时针刺开音1号穴及合谷穴,每日1次,每次15分钟.7天一个疗程,连续两个疗程.结果中西医结合组76例中,声带小结总有效率100%.声带息肉总有效率96%.西医组40例中,声带小结总有效率80%,声带息肉总有效率76%,两者进行统计学分析差异有显著性意义(P＜0.01).结论针药加微波热凝治疗声带息肉、小结具有各自疗法的优点,同时克服了不足之处,且疗效好、痛苦小、副作用少、复发率低.Objective To investigate the therapeutic effects of a combined therapy with acupuncture, herbal medicines and microwave thermocoagulation locally applied for the treatment of polyp and/or nodules of vocal cord. Methods Microwave thermocoagulation was applied into the polyp or nodule tissue via a soft wire electrode through fibrolaryngoscope at first. Then, gentamycin, dexamethasone and chymotnypsin were postoperatively given in the way of ultrasonic inhaling, twice a day, and acupuncture was supplemented once a day to the points of Kaiyin 1 and Hegu lasted for 14 days. Results The curative rate was 81% for vocal cord nodules and 78% for polyp of vocal cord, with the total effective rates being 100% and 96% for them respectively, significantly higher than those of controlling group (being 80% and 76% respectively, P<0.01). Conclusions The combined therapy with acupuncture, herbal medicines and microwave thermocoagulation iseffective for the treatment of polyp and nodules of vocal cord.
