母乳喂养对减少糖尿病产妇胰岛素用量的作用

摘　　要：对我院１９８７年１月～１９９３年６月，胰岛素依赖型糖尿病（ＩＤＤＭ）产妇３７例和需要在妊娠期应用胰岛素（ＲＩ）治疗的妊娠期糖尿病（ＧＤＭ）１０例，共４７例进行分析。对她们在孕前、妊娠期、产后哺乳期和目前ＲＩ用量，分母乳喂养组和退奶组进行了对比研究。结果表明，ＩＤＤＭ产妇母乳喂养组和退奶组在孕前、妊娠期和目前ＲＩ用量尤差别，唯有产后哺乳期ＲＩ需要量在母乳喂养织明显少于退奶组，经ｔ检验，两织差异有显著性。１０例ＧＤＭ孕妇妊娠期需ＲＩ治疗，退奶组３例产好产后继续需要ＲＩ治疗４～７天，而母乳喂养组则不需要ＲＩ治疗，血糖均在正常水平。通过分析可初步提示如下：哺乳可以减少糖尿病产妇产后ＲＩ的用量，因此应该鼓励糖尿病产妇进行母乳喂养。rom Jan. 1987～Jun. 1993, 37 cases of insulindependent diabetes mellitus(IDDM pregnant womenand 10 gestational diabetes mellitus (GDM)mothersrequiring regular insulin(RI) treatment during preg-nancy were recruited.A comparative-study of the dailyRI requirement was carried out in women breast-feed-ing(BF) or not(non-BF)at different periods:before pregnancy,during gestation, postpartum and at pre-sent.Results showed no difference of RI requirementbetween BF and non-BF groups before pregnancy,during gestation and currently,but a significant de-crease of RI requirement(P<0.05) among BF moth-ers in postpartum period. Of the 10 GDM cases, 3 ofthem who did not breast feed their babies required fur-ther RI for 4～7 days in the postpartum period,Whereas 7 BF mothers did not need any RI after deli- very with blood glucose levels remaining within nor-mal range.Based on the above analysis one may concludethat BF can reduce the RI requirement of IDDM andGDM mothers in the postpartum period. It is thoughtthat more energy is needed in the process of producingmilk, and serum glucose is the main substance forlactose synthesis,thus blood glucose level of BF moth-er is decreased, and so is the Rl requirement. There-fore, mothers with diabetes mellitus are encouraged tobreast keed their babies.
