摘 要:目的:探讨传统中草药和新疆维吾尔药共同组方的复方制剂心痛宁加味方对急性实验性心肌缺血大鼠的疗效及作用机制。方法:65只大鼠随机分为假手术组、生理盐水对照组、复方丹参片组、心痛宁加味方大、中、小剂量组。末次给药后1h制模。采用冠状动脉结扎法造成大鼠急性心肌缺血模型。检测各组大鼠冠状动脉结扎后不同时间点心电图ST段抬高程度、心肌梗死范围及血浆一氧化氮(NO)和内皮素(ET)水平。结果:心痛宁加味方对大鼠实验性急性心肌缺血有明显的改善作用,与复方丹参片组比较,可明显降低心电图ST段的抬高程度,有效缩小心肌梗死范围。急性心肌缺血时,血浆NO降低、ET明显升高;心痛宁加味方能明显提高大鼠急性心肌缺血时NO水平,降低ET水平,以大剂量组效果最为明显。结论:心痛宁加味方抗心肌缺血的作用机制可能与调节NO和ET平衡、保护血管内皮损伤有关。Objective: To investigate the mechanism and therapeutic effect of modified Xintongning prescription(心痛宁加味方) whose compound formula is the combination of traditional Chinese herbs and Xinjiang Uygar drugs on rats with experimental acute myocardial ischemia. Methods: Sixtyfive rats were randomly divided into following groups: sham operation, normal saline(NS), composite salvia miltiorrhiza tablet and low, middle, high dosage of modified Xintongning prescription groups. The rat model of acute myocardial ischemia was established by ligating the coronary artery 1 hour after administration of drug. At different time points after the ligation, the following items were investigated: the height of ST segment, sizes of myocardial infarction zone, the plasma content of nitric oxide(NO) and endothelin(ET). Results: The effect of modified Xintongning prescription was superior to that of composite salvia miltiorrhiza on reducing the increased extent of ST segment and diminishing the size of myocardial infarction zone. NO was decreased and ET was increased in rats with acute myocardial ischemia. Modified Xintongning prescription could increase NO level and decrease ET level in the rats with acute myocardial ischemia, especially in largedose of modified Xintongning prescription group. Conclusion: The mechanism of modified Xintongning prescription is possibly due to regulating the balance of ET and NO and protecting vascular endothelial cells from injury.
