摘　　要：目的探讨经阴道超声(TVU)在评价二甲双胍治疗胰岛素抵抗(IR)的多囊卵巢综合征(PCOS)疗效中的价值。方法利用TVU对39例胰岛素抵抗的多囊卵巢综合征(PCOS-IR)妇女在二甲双胍治疗前后的卵巢结构(卵巢面积TA、间质面积SA、SA/TA卵泡计数)进行对比观测,并测定生殖激素:促黄体生成素(LH)、卵泡刺激素(FSH)、睾酮(T)及空腹血糖(FBS)和空腹胰岛素(FIN)水平。结果治疗后LH、LH/FSH、T、IR显著降低,TA、SA、SA/TA及卵泡计数减少,统计学上差异有显著性。相关分析显示:SA/TA及卵泡计数的变化与激素的变化相关性良好。结论TVU通过将SA/TA及卵泡计数结合可有效评价二甲双胍疗效。[Objective] To investigate the clinical value of transvaginal ultras ound(TVU) in polycystic ovarian syndrome accompanied by insulin resistance(IR) w hich was treated in metformin. [Methods] The features of ovarian morphology(st romal area SA﹑total area TA)and the number of follicles of 39 cases with PCOS-I R were detected by TVU; the levels of LH, FSH, T, fasting blood-glucose (FBS) an d fasting serum insulin(FIN) were tested; And the SA/TA, LH/FSH and IR were calc ulated before and after metformin administration. [Results] The levels of LH, LH/FSH, T and IR were decreased significantly (P <0.05); The number of follicles dropped significantly; The changes of the number of follicles and parameters of ovarian morphology were analyzed by linear correlation with the change of IR. T he changes of the number of follicles and SA/TA had significant correlations wit h the changes of IR and generative hormone respectively. [Conclusion] TVU is u seful to assess the effect of metformin by combining SA/TA with the number of fo llicles.
