肺结核与支气管哮喘并发支气管粘液嵌塞X线分析(附七例报告)

摘　　要：目的：探讨肺结核与支气管哮喘并发支气管粘液嵌塞的发病机理,临床与X线特征。材料和方法：对7例肺结核与支气管哮喘并发支气管粘液嵌塞的临床及X线表现进行分析。结果：该病好发于右肺上叶支气管、粘液痰栓和肺不张在X线片上表现如墨鱼状、粘液痰栓相当于墨鱼的足─—呈V、Y或树枝状影,肺不张呈块状影,相当于黑鱼头与体部,有时可观察到痰柱周围的气体形成包壳状影。当粘液栓咳出后、肺部分或完全复张、可显示与粘液栓形态一致的支气管气象及扩张支气管的轴位象。结核患者尚可显示原有的结核灶。结论：肺结核与支气管哮喘并发支气管粘液嵌塞具有特征性的临床及X线表现。Purpose: To explore the pathogenestic mechnisms,clinicai and x - ray characteristicsin pulmonary tuberculosis and bronchial asthma complicated with mucoid impaction.Materialsand methods:Seven cases of pulmonary tuberculosis and bronchial asthma complicated with mu-coid impaction in bronchi, the clinical and x-ray manifestations are analysed. Results:In plainchest film,the mucoid impaction is liable to occur in the right upper bronchus,the signs of x-rayof the plug of impacted mucus and pulmonary atelectasis show as the outlook of ink fish,the mu-coid plug correspond to the foot of ink fish,manifested as V,Y shape or branching shape,and theatelectasis as masses shadow,correspond to the head or body of the ink fish,the shell - like gasshadow around the plug are also observed. After the plug coughed up,the lung may be partial orcomplete re - expansion. The mucoid plug morphology usually exhibited consistence withbronchial ari-imaging and the axis-imaging.In TB patients the original tuberculosis foci mayalso be exhibited. Conclusion:The pulmomary tuberculosis and bronchial asthma complicated withbronchial mucoid impaction posses characteristic clinical and x-ray manifestations.
