76例老年妇女妇科手术的临床分析

摘　　要：分析７６例老年妇女妇科手术的临床资料，占同期各类妇科手术的４．４％。其中６２例（８１．６％）为妇科肿瘤，恶性肿瘤３４例，占５４．８％。术前有各种并存症者５９例（７７．６％），以心血管疾病为最多（５６．６％），其次是呼吸系统疾病（２３．７％）。采用连续硬膜外麻醉７４例，鞍麻及骶麻各１例；１例发生麻醉意外。腹部手术６５例（８５．５％），阴道手术８例，外阴手术３例；大中型手术平均手术时间２小时２０分钟，无手术意外及死亡。术后发生并发症者１２例（１５．８％），死亡１例。我们认为只要适当地选择病例，实施妥善的围手术期处理，老年妇女是能很好地耐受手术的。Abstract Seventy six cases of gynecological operations in elderly over 60, accounted for 4.4% of total cases of gynecological operations performed during the period of 1985-1992 were analyzed. Among the 76 cases, 62(81.6%) were gynecological tumors, of which 34 cases(54.8%) were malignant tumors. Fifty nine cases(77.6%) had one or more concomitant medical disorders of clinical importance. Most(56.6%) of them had cardiovascular diseases and more(23-70%) had respiratory disorders. Continuous and epidural anesthesia was used in 74 cases, with one accident. Transabdominal operation was performed in 65 cases(85.5%). transvaginal in & cases and vulval in 3 cases. The mean operative time was 2 hours and 20 minutes. No operative accident had happened in this group. The rate of postoperative complications was 15.8%, one of them died.We believe that the aged women can tolerate gynecological surgery well if the case is Properly selected and careful peri-operative management is instituted.
