哮喘急性发作患儿血清COX-2动态改变的临床意义

摘　　要：目的:了解哮喘急性发作患儿血清环氧化酶-2(COX-2)变化的临床意义及与病情的关系。方法:采用ELISA方法动态观察了50例哮喘发作患儿急性期和缓解期及20例正常儿童血清COX-2的含量。结果:哮喘发作患儿血清COX-2水平急性期[(37.09±4.63)IU/L]>缓解期[(16.28±3.97)IU/L]>对照组[(7.32±2.26)IU/L],P均<0.01。急性期患儿血清COX-2含量分别与外周血白细胞总数[(12.36±2.73)×109/L]呈明显的正相关性(r=0.591,P<0.01)、与动脉血气中PaO2(5.85±3.67)kPa和pH值(7.28±0.29)均呈显著负相关(r=-0.714和-0.808,P均<0.01)。结论:COX-2可能参与了哮喘急性发作患儿的发病过程,动态测定患儿血清COX-2的含量,对监测患儿的病情变化及严重程度可能有一定的意义,拮抗COX-2可能有助于缓解患者的病情。Objective:To investigate the potential effect of levels of serum COX,2 in the asthma children. Methods: The levels of COX,2 in 50 pediatric patients with asthma and 20 normal children were dynamically measured by ELISA. Results: The results showed that serum COX,2 in the asthma children during acute phase [(37.09±4.63)IU/L] > those in the patients during recovery phase [(16.28±3.97)IU/L]> those in the normal controls[(7.32±2.26)IU/L], respectively, all P < 0.01. Moreover, there were significantly positive correlations of the levels of serum COX,2 with the levels of WBC counts [(12.36±2.73)×109/L] in the patients during acute phase(r=0.591,P < 0.01). There were significantly negative relationships in the COX,2 levels with PaO2(5.85±3.67)kPa and pH (7.28±0.29) in the patients (r=-0.714 and -0.808, all P < 0.01, respectively). Conclusions: The data suggested that COX,2 may play an important role in the pathogenesis of the inflammatory process of asthma in the pediatric patients, and that measuring the serum COX,2 levels may be helpful in monitoring the changes of the disease. The therapy of anti,COX,2 may relieve the condition of the asthma patients.
