摘 要:目的探讨磁共振内耳膜迷路成像的技术及临床价值。方法利用西门子公司1.0TMAGNETOMIMPACT超导型MR扫描机软件版本VB33D,采用梯度回波序列中的三维镜像稳态快速成像序列3D-PSIF,对1例疑有内耳疾病患者和20例健康志愿者作MR内耳膜迷路成像,所有患者均先行常规MR扫描,并行最大信号强度投影MIP及多平面重建MPR后处理。结果10例先天性感音性耳聋患者中,包括功能性毛细胞及器质性耳硬化等畸形;20例健康志愿者中,17例均能满意显示双侧内耳膜迷路及内听道的精细解剖结构,2例显示不佳,1例无法显示。结论内耳膜迷路MR成像为一种有效且无创的影像检查方法,它不仅可以显示内耳膜迷路、内听道的立体解剖结构,而且能够清晰显示其通畅程度,这样就可以帮助临床耳科医生分析病情,同时为内耳外科手术,尤其是电子耳蜗植入等手术提供重要信息,并在一定程度上可排除一些绝对或相对禁忌症,大大提高手术的准确性。Objective To investigate the use of magnetic re sonance(MR)imaging technique in inner ear membr anous labyrinch.Methods The study was performed on Siemens 1.0T MAGNETOM IMPACT Superconductive MR -scanner(software VB33D)with 3D -PSIF of gradient echo sequen ce.Ten patients with suspected inner ear diseases and twenty healthy volunteers underwent MR imaging in inner ear membranous labyrinch and post -processing by maximu m intensity projection(MIP)and multi -planar reconstruction(MPR)after conventional MR Scans.Results Ten patients with congenital sensory deafness included functional(hair cell)deafness and or-ganic deafness(otosclerosis malformation).Of 20healthy volunteers,the fine a natomic structures within bilatera l inner ear membranous labyrinch and internal acoustic meatus were clearly revealed in 17cases,unclearly revealed in 2cases,and b e unable to reveal in 1case.Conclusion MR imaging technique in inner ear mem branous lobyrinch is a non -invasive examination.It can' t only reveal the stereoanatomic structures within i nner ear membranous labyrinch and in ternal acoustic meatus but can directly reveal their passage state so that otologic doctor can sufficient ly understand the local lesions and o btain important informations in relation to whether it needs inner ear surgery or implant of electronic coc hlea.This technique can also exclude the absolute or relative contraind ications from operation and greatly improve the operative accuracy.[
