腰椎间盘突出症再次手术的原因分析

摘　　要：目的　　解决腰椎间盘突出症手术中神经压迫。方法　对 1980～ 1998年再手术者资料进行统计分析 ,讨论分析再手术原因 ,再次手术前影像学检查 ,观察病理变化以确定再手术方法。结果　对 11例随访 6个月～ 1年 ,优 7例 (68.4 % ) ,良 3例 (36.8% ) ,差 1例 (2 .8% )。结论　初次手术前详细查体和分析X线片 ,术中用导尿管和神经剥离探查 ,尽量避免髓核遗留 ,手术范围不宜太大 ,尽量减少对软组织和脊柱结构的破坏 。Objective In order to resolve the pressure problems of nerve roots in intervertebral disc operation.Methods 11 cases of protrusion of the intervertebral disc have been reoperated in our hospital,finding out the results,and observed the pathological changes before reoperation.Results From six months to one year,we investgated 11cases,the conditions were as follows:7 cases were good (68.4%),3 cases were better (36.8%),1 case was bad (2.8%).Conclusion There are some causes below:l.The position of movable spinal column had fixed in mistake so that had not fixed accurately. 2 The spinal column side hidden lpt can not remove the nerve root of pressure phenomenon.3.Lumbar intervertebral disc has not found out or protruded later also can not remove the nerve root of pressure. 4. The operation has not performed carefully and pathological changes have not found out so that it has not removed the pressure of nerve root. 5. The stable structure of spinal column has been damaged and it is one of the major cause of a relapes of protrusion of the intervertebral disc. So during the operation of protrusion of intervertebral disc,the pressure problem of nerve rootshould be solved,otherwise it is quite difficult to avoid a relapse.
