摘 要:目的探讨人类白细胞抗原(HLA)—氨基酸残基配型在肾移植中的临床应用价值。方法应用HLA氨基酸残基配型(ResM)标准及传统的六抗原配型标准(AgM)。分析在尸肾移植中HLA——氨基酸残基配型对肾移植结果的影响。结果采用ResM标准可大幅度提高相配几率,0、1mm组受者由1.8%提高到42.9%;2mm组受者由1.8%提高到41.1%。结论新的ResM标准具有实用性与可行性,可大幅度提高移植供受者的相配几率,适合于肾移植的临床应用。[Objective] To study the clinic application of HLA-amino acid residue matching in kidney transplantation. [Method] HLA-amino acid residue matching (ResM) and HLA six-antigen match program (AgM) were used to analyse the influence of HLA-amino acid residue of cadaver kidney transplantation. [Result] The rate of Res-matched transplants increased greatly. The rate of 0~1 mm recipients increased from 1.8% to 42.9%, and that of 2 mm transplants from 1.8% to 41.1%. [Conclusion] New ResM has been proved to be suitable and feasible, and can greatly enhance the rate of transplants, which is suitable for clinical application in kidney transplantation.
