运动心电图试验最大ST/HR斜率对冠心病的诊断价值探讨

摘　　要：目的:探讨运动心电图试验中最大ST/HR斜率对冠心病的诊断价值。方法:将54例患者根据冠状运动造影结果分为冠心病组及对照组,观察在运动心电图试验时最大ST/HR斜率值及ST·215·段压低值。结果:最大ST/HR斜率对冠心病诊断的敏感性(91.43%)和特异性(89.47%)均优于普通运动心电图试验(分别为82.86%、63.16%),两组比较差异有显著意义(p<0.05)。最大ST/HR斜率法对单支冠脉病变血管的检出敏感性高于ST段压低法(分别为80.95%、57.14%),两组比较差异有显著意义(p<0.01),对双支及三支冠脉病变血管的检出敏感性二者相同(均为100%)。结论:最大ST/HR斜率对诊断冠心病有重要价值,其敏感性及特异性均明显高于ST段压低法,并对预示冠状血管病变数目有一定的价值。Objective: To investigate the clinical diagnostic value of the maximal ST segment/heart rate slope(ST/HR slope) in patients with coronary heart disease(CHD). Method: The ST/HR slope and ST segment depression value were studied in 35 patients with confirmed CHD by coronary angiography,and 19 healthy controls suspected CHD underwent coronary angiography, during the exercise electrocardiographic testing. Result: The ST/HR slope was more sensitive and specific than the ST segment depression(91.43%,89.47 % vs 82.86%,63.16 %),and the sensitivity of diagnosis of the diseased coronary artery vessels were 80.95% and 57.14% in 1 branchs, and no significant difference between two groups in 2～3 branches. Conclusion: There is important value in diagnosis of CHD by the ST/HR slope, whose sensitivity and specificity are significant higher than common ST segment depression .And it has also predictive value on the number of the diseased arteriosclerostic coronary artery vessels.
