摘　　要：目的研究小儿先天性肾积水血、尿β2-微球蛋白(β2-microglobulin,β2-MG)测定对评价肾功能损害的意义。方法通过放射免疫法测定先天性肾积水患儿和正常儿童血、尿β2-MG的水平,比较观察组与对照组之间血、尿β2-MG水平差异。结果轻度肾积水患儿尿β2-MG含量较对照组明显升高,但血清β2-MG水平与对照组比较无显著性差异,中、重度肾积水患儿血、尿β2-MG含量与对照组比较明显升高。不同程度肾积水患儿间血、尿β2-MG含量比较有显著性差异。结论尿β2-MG是反映肾小管功能的灵敏指标,轻度先天性肾积水患儿尿β2-MG含量明显升高,提示轻度肾积水患儿已存在肾小管功能损害,血β2-MG含量较对照组无明显升高提示肾小球功能尚在正常范围,轻、中、重度肾积水患儿血、尿β2-MG含量有显著差异,提示不同程度肾积水肾功能损害程度不同,本研究证明,血、尿β2-MG水平对反映小儿先天性肾积水造成的肾功能损害具有较高的敏感性。Objective To study the value of detecting β2-microglobulin to evaluate kidney damage in children with congenital hydronephrosis. Methods The β2-microglobulin in serum and urine in children with congenital hydronephrosis and in healthy children were determined by radioimmunoassays,to compare the levels of β2-microglobulin in congenital hydronephrosis with that in healthy children. Results The level of β2-microglobulin in urine in children with light hydronephrosis was higher obviously than that in healthy children,but there was no difference between light hydronephrosis group and control group in the level of β2-microglobulin in serum. The level of β2-microglobulin in urine in children with moderate and severe hydronephrosis groups was higher significantly than that in control group.The difference of β2-microglobulin among the three hydronephrosis group was significant. Conclusions The level of β2-microglobulin in urine is a sensitive marker to reflect the function of renal tubule. The Urinary β2-microglobulin remarkable increasing level in light degree hydronephrosis might correctly reflect the damage of renal tubule,but the β2-microglobulin in serum doesn't increase significantly,which indicates that the function of renal glomerulus is in normal range. The different levels of β2-microglobulin in serum and urine of children with mild,moderate and severe hydronephrosis indicates that the damage degree of the kidney function is different, This study suggests that the level of β2-microglobulin in serum and urine could sensitively reflect the damage of the kidney function.
