摘  要:我们采用在门静脉、肠系膜下静脉和中心静脉插入导管,分别测定其葡萄糖含量并结合门静脉血流量测定使其表达量化的方法,对严重创伤后肠道吸收功能的变化进行了研究。实验结果表明,影响伤后葡萄糖吸收的主要原因是门静脉系血循环障碍;而肠道本身的吸收能力并没有大幅度减少。这提示,如能及时有效地改善门静脉系血循环状况,肠道吸收功能可明显改善并在较短时间内恢复至伤前水平,以往常规的抗休克疗法并不能及时有效地改善门静脉系血循环状况。实验还表明早期肠道营养是达到此目的的一个有效手段。AbstractWe investigated the changes of glucose absorptionand the effect of earlv enteral nutrition after severeburn injury。Anew animal model was developed with aspecially bred miniswine(Gui Zhou species)to whichmuItiple catheterizations were applied for sampling dif-ferent blood from the portal, inferior mesenteric aswell as jugular (central)veins; The animals were sus-tained with 30%III burns of TBSA 7 davs after thecatheterizations and divided randomly into early feeding(EF ) group, given a complete diet beginning from 2hours postburn, and delayed feeding(DF)group, giv-en the same diet on 4 days postburn, After burn in-jury, glucose absorption was significantly decreased,but the absorption was much lower in group DF thanthat in group EF. However, the intestinal abilitv of ab-sorbing nutrients was not attenuated greatly, and thereduction of portal blood flow was attributed to the de-crease of absorptiOn of nutrients. Early enteral feedingcan increase portal blood flow obviously.
