摘  要:目的研究高压氧预治疗对颈椎管狭窄术后脊髓神经根性水肿的预防、治疗作用,并探讨其机制。方法选择各种原因导致的颈椎管狭窄患者90例,随机分为对照组、高压氧预治疗组及高压氧治疗组,每组各30例。对照组直接行常规手术,高压氧预治疗组术前给予高压氧治疗1周,高压氧治疗组术前及手术3d后各给予高压氧治疗1周,余治疗同对照组。所有患者于术前及术后1d、7d、1个月和1年采用改良的JOA40分法进行评分,并检测诱发电位(SEP)。结果2个治疗组术后7d、1个月、1年改良的JOA评分及SEP检测结果(N20、P25)与对照组相应时间点比较,差异均有统计学意义。结论术前高压氧治疗有助于预防术后脊髓再灌注损伤及反跳性水肿。Objective To study the preventative and therapeutic effects of hyperbaric oxygen (HBO) on reperfusion injury and edema of spinal cord after operation of cervical spinal stenosis (OCSP), and to explore its mechanism. Methods Ninety patients with cervical spinal stenosis were randomly and equally divided into a HBO1 group, a HBO2 group and a control group. All patients were treated with OCSP, and patients in HBO1 group were treated with HBO before operation, patients in HBO2 group were treated with HBO before and after operation. JOA scores and SEP were evaluated before, and 1 day, 7 days, 1 month and 1 year after operation. Results Compared with those in the control group, JOA scores and SEP in HBO1, HBO2 groups were significantly different (P<(0.05)). Conclusion Preoperative HBO therapy could be helpful to prevention of the reperfusion injury and edema of spinal cord after operation.
