新型农村合作医疗制度对农民医疗费用的影响研究

摘　　要：目的:了解新型农村合作医疗实施1年后,农民的门诊与住院医疗费用变化情况,评价新型农村合作医疗实施对医疗费用的影响。方法:采用分层整群随机抽样方法抽取婺源县3个乡镇9个行政村,每村70户入户进行基线调查和1年后的跟踪调查;同时随机抄录乡、县两级医院部分农民的住院费用情况进行分析。结果:新型农村合作医疗实施1年后婺源县农民年人均医疗费用为2 71.87元,较基线调查的2 12 .6 2元高出5 9.2 5元,其中门诊费用降低了0 .0 5元,住院费用升高了5 9.30元。住院费用增高主要是2 0 0 0 0元以上费用段费用增高所致。结论:婺源县在实施新型农村合作医疗制度1年后,农民年人均医疗费用升高,主要表现为大额住院费用的升高,应采取相应对策。Objective to get some idea of farmers clinic and hospitalization cost change condition,after new- type rural cooperative medical system( CMS) was carried outone year,to appraise the impact of the new- type rural CMS on medical cost.Methods We adopted stratified cluster random sampling to choose 3townships ,9administrative villages in Wuyuan county,70 households in every village,and carried outthe base- line investigation and follow- up investigation.We made a copy of some farmers'medical expenses in township and county hospitals at random atthe same time and analysed.Result The annual per capita medical cost of farmers in Wuyuan county is 2 71.87yuan after new- type rural CMS was implemented one year,which is 5 9.2 5 yuan higher than the base- line investigation’s 2 12 .6 2 yuan.Among the annual per capita medical cost ,clinic expenses have been reduced by 0 .0 5 yuan and expenses in hospital have raised 5 9.30 yuan.The expenses increased in hospitalization is mainly because that expenses in the category of more than 2 0 0 0 0 yuan increase.Conclusion The annual per capita medical costof farmers in Wuyuan county increased after new- type rural CMS was implemented one year.The increase is represented by the raising of large amount cost for hospitalization,so we should take some corresponding countermeasure to control it.
