经皮二尖瓣球囊扩张术对右心功能的影响

摘　　要：对４０例经皮二尖瓣球囊扩张术患者术前与术后的右心功能指标进行比较。结果表明，术后患者的右室射血分数比术前明显升高，收缩和舒张末期容积缩小；右室射血分数的变化与二尖瓣口面积的改变呈正相关。Abstract The right ventricular function was studied before and after percutaneous balloon mitral valvuloplasty(PBMV) in 40 patients with rheumatic mitral stenosis.Right ventricular volumes and right ventricular ejection fraction(RVEF) were estimated by two-dimensional echocardiography from single-plane area-length method on apical four -chamber view. The results showed that RVEF was significantly increased 24 hours after PBMV (56. 199±7. 237%) compared with that before PBMV (47. 054 ± 7. 671% , P<0. 001). Both right ventricular end-diastolic volume index(RVEDVI) and end-systolic volume index (RVESVI) were decreased after PBMV(RVEDVI, 78. 007 ± 19. 732; RVESVI, 41. 437 ±12. 899ml/m2) compared with those before PBMV (RVEDVI, 65. 664±17. 364 and RVESVI, 29. 089±10. 103ml/m2, both P < 0. 001); the degree of RVESVI decrease (29. 48±12. 69%) was more marked than that of RVEDVI (15. 19±11. 01% , P<0. 001).However, right ventricular stroke volume index changes were not significantly different before and after PBMV.We concluded that PBMV significantly improved right ventricular function in patients with rheumatic mitral stenosls.
