摘　　要：目的介绍腹腔镜直肠癌根治术的一种经济可行的手术方式。方法腹腔镜下手术操作与传统开腹手术方式步骤相同,采用拉下式直肠吻合技术。不使用价格昂贵的进口吻合器及切割器,大大的节省了手术费用,使腹腔镜手术在经济不发达地区也能广泛开展。结果5例患者恢复良好,术中无脏器损伤,平均出血量150 ml。住院时间平均12.6 d。生活质量好,无复发转移。手术费用与常规手术无明显差别。结论腹腔镜直肠癌根治术可以达到传统手术的疗效,甚至有一定优势。但必须严格掌握其手术适应证,手术技术要求高,既需要丰富的开腹手术经验,又必须有扎实的腔镜操作能力。避免盲目或勉强镜下完成手术。Objective: To introduce an economical and practical method of laparoscopic colectomy for rectal cancer. Methods: The method we used in the laparoscopic operation was to adopt the ano-rectum anastomosis through anus which remains the same as the traditional laparotomy. Not using expensive imported Staples and GIA reduced the operation expenses greatly which makes laparascopic operation possible and even popular in the economically less developed regions. Results: Five patients recovered well and there was no viscera injury during the operation. The average blood loss was 150 ml. The average hospital stay was 12.6 days. The life quality was good and there was no recur metastasis. There was no obvious difference in the operation expense with the traditional laparotomy. Conclusion: Laparoscopic colectomy for cancer can achieve the same medical treatment effect as the traditional laparotomy and even better. But strict attention should be paid to the suitable situation of the operation. It challenges the surgeons with high skills, rich experience of laparotomy as well as solid competence of laparoscopic operation so as to avoid blind or reluctant laparascopic operation.
