颈动脉灌注红花注射液治疗脑梗死的临床研究

摘　　要：目的:探讨颈动脉灌注红花注射液治疗脑梗死的速效、量效关系与安全性。方法:将250例脑梗死患者随机分为红花注射液颈动脉灌注A、B、C、D4个亚组(治疗组)各50例和红花注射液静脉滴注组(对照组)50例,4个亚组:A组(滴速3mL·min-1,剂量10mL/次);B组(滴速3mL·min-1,剂量20mL/次);C组(滴速4mL·min-1,剂量10mL/次);D组(滴速4mL·min-1,剂量20mL/次);20d为1疗程。观察治疗前后语言、肢体肌力、综合自理能力及血液流变学变化。结果:(1)治疗组总有效率为80.0优于对照组62.0(P<0.01)。(2)治疗组分为4个亚组有效率分别为:A组66;B组76.0;C组84.0;D组94。其中以4mL·min-1,20mL·次-1大剂快滴的D组效果最明显,与对照组比较差异有显著意义(P<0.01)。结论:运用颈动脉灌注红花注射液治疗脑梗死对患侧肢体功能恢复等疗效优于静脉全身给药的对照组,灌注理想有效剂量为20mL·次-1,理想安全速度为4mL·次-1。AIM: To observe the clinical efficacy and safety with the dose response relationship in the treatment of cerebral infarction by Safflower Injection via carotid artery infusion. METHODS: 250 cases were randomized into A、B、C、D four groups(treatment group), each one was 50 cases treated with Safflower Injection via carotid (artery) infusion and control group in which 50 cases were treated with intra venous drip of Safflower Injection. The four groups: group A (infusion velocity 3 mL/min, dose 10 mL/time); group B (infusion velocity 3 mL/min, dose 20 mL/time); group C (infusion velocity 4 mL/min, dose 10 mL/time); group D (infusion velocity 4 mL/min, dose 20 mL/time); at a course of 20 days. The speech function myodynamia、self-care ability and hemorrheology were compared between before and after treatment. RESULTS: (1) The total effective rate was (80.0)% in treatment goup, higher than (62.0)% in control group (P<(0.01)). (2) The group A of effective rate was (66.0)%, group B (76.0)%, group C (84.0)% and group D 94%, respectively. The outcome in the “highest MAP largest dosage”(group D) was the best and was obviously superior to that in control group (P<(0.01)). CONCLUSION: It is suggested that Safflower Injection via carotid artery infusion is better than intra venous drip in the treatment of cerebral infarction. The best dosage is 20 mL/time and the best velocity is 4 mL/min.
