乙型肝炎病毒基因型在无症状感染者、慢性乙肝和肝硬化患者中分布研究

摘　　要：目的探讨乙型肝炎病毒基因型与HBV感染结局之间存在关联。方法采用病例-对照研究方法,征集300例无症状感染者、668例慢性乙肝患者和108例肝硬化患者作为研究对象,应用聚合酶链反应(PCR)方法对乙型肝炎病毒基因型进行测定。结果在无症状携带组、慢性肝炎组和肝硬化组中,HBVDNA阳性率分别为42·7%、91·0%和50·0%。无症状携带组HBV基因分布分别为B型28·1%、C型68·8%和混合型3·1%。慢性乙型肝炎为B型20·9%、C型72·0%和BC混合型7·1%。肝硬化组为B型11·1%、C型83·3%和混合型5·6%。不同组别的HBV病毒基因型构成比不同(χ2=9·981,P=0·041)。结论HBV病毒基因型可能是影响HBV感染结局的重要因素。Objective To observe distribution of hepatitis B virus (HBV) genotypes in asymptomatic HBV carriers, chronic HB patients, and liver cirrhosis, and to infer association of HBV genotypes with outcomes of HBV infection. Methods A total of 300 asymptomatic carriers,668 chronic hepatitis B patients, and 108 patients with liver cirrhosis were recruited,and their HBV genotypes were measured by polymerase chain reaction(PCR). Results The positive rates of HBV DNA in asymptomatic carrier group, chronic HB patient group and liver cirrhosis group were 42.7%, 91.0% and 50.0%, respectively. HBV genotype distribution among the asymptomatic carriers was 28.1%, 68.8%, and 3.1% for genotype B, C and mixed infection, respectively,and 20.9% for genotype B,72.0% for genotype C, 7.1% for mixed genotype in chronic HB group, and 11.1% for genotype B, 83.3% for genotype C and 5.6% for mixed infection in liver cirrhosis group. A statistically significant difference in genotype distribution was found among the three groups(χ~2=9.981,P=0.041). Conclusion There is a significant difference in the distribution of HBV genotypes among symptomatic carrier group,chronic HB patient group and liver cirrhosis group, HBV genotypes are possibly playing an important role in influencing outcomes of HBV infection.
