石家庄市健康中学教师100人动态血压随龄的变化

摘　　要：目的:探讨健康中学教师动态血压变化规律。方法:选择2000-04/06石家庄市藁城中学健康中学教师100人,男45人,女55人,年龄21～60岁。采用Spacelabs动态血压监测仪对受试者进行24h动态血压监测。受试者年龄21～60岁,每10岁为一年龄组,21～30岁组32人,31～40岁组25人,41～50岁组20人,51～60岁组23人。记录收缩压、舒张压、平均动脉压、心率;计算血压负荷。动态血压均数正常参照值:24h低于130/80mmHg(1mmHg=0.133kPa),白昼低于135/85mmHg,夜间低于125/75mmHg;血压负荷(在一定时间内收缩压和舒张压超过正常范围的读数百分率)低于10%。结果:按意向处理分析,进入结果分析中学健康中学教师100人。①21～30岁组,31～40岁组,41～50岁组24h及白昼、夜间平均收缩压、平均舒张压、平均动脉压无明显差异(P>0.05)。②51～60岁组24h平均收缩压、平均舒张压及平均动脉压明显高于21～30岁组,31～40岁组,41～50岁组犤(119±4),(76±5),(90±4)mmHg;(110±7),(68±5),(82±4)mmHg;(112±8),(70±4),(84±4)mmHg;(112±5),(71±6),(86±4)mmHg,P<0.01)犦。③51～60岁组白昼平均收缩压、平均舒张压及平均动脉压明显高于21～30岁组,31～40岁组,41～50岁组(P<0.01)。④夜间平均收缩压:41～50岁组,51～60岁组平均收缩压、平均舒张压及平均动脉压明显高于21～30岁组,31～40岁组犤(108±8),(108±7)mmHg;(105±8),(106±9)mmHg,P<0.01犦;夜间平均舒张压:51～60岁组明显高于21～30岁组,31～40岁组,41～50岁组犤(69±5),(63±4),(64±5),(65±7)mmHg,P<0.01犦。⑤各年龄组24h、白昼、夜间舒张压负荷明显高于收缩压负荷(P<0.01);夜间收缩压和舒张压负荷明显高于白昼(P<0.01)。结论:健康中学教师24h及白昼平均收缩压、平均舒张压、平均动脉压有随年龄增高而增高的趋势;21～50岁教师血压增高不明显,50～60岁教师血压增高明显。健康中学教师的动态血压虽在正常范围内,但夜间血压负荷与�AIM:To investigate the changing regularity of ambulatory blo od pressure in healthy teachers of middle school.METHODS:A total of 100healthy teachers of mid dle school (45males and55females)at 21to 60years of age were selected from Gaocheng Middle School in Shijiazhuang city between Apri l and June 2000.The 24-hour am-bulatory blood pressure of the subje cts was monitored with the Space labs ambulatory blood pressure monitor.The 21to 60-year-old subjects were d i-vided into groups with every 10years old a s an age group:21to 30-year-old group(n=32),31to 40-year-old group(n=25),41to 50-year-old group(n =20),and 51to 60-yea r-old group(n =23).The systolic pressure,dias-tolic pressure,mean arte rial press ure and heart rate were recorded,and blood pressure load was calcula ted.Norma l reference values of the ambulatory blood pressure were lower than 130/80mm Hg (1mm Hg =0.133kPa)for24hours,lower than 135/8 5mm Hg for day,and 125/75mm Hg for night;blood pressure load(the perce ntage of reading number of s ystolic pressure and diastolic pressure exceeding t h e normal range within a certain durat ion)lower than 10%.RESULTS:Accordi ng to intention-to-treat an alysis,all the 100healthy teachers of middle s chool were invol ved in the analysis of results.①There were no obvious differe nces in the 24-hour,diurnal and nocturnal mean sy stolic pressure,mean dias tolic pressure a nd mean arterial pressure among the 21to30,31to 40and 41to 50-year-old grou ps(P>0.05).②The 24-hour mean systolic pr essure,mean diastolic p ressure and mean arterial pressure were ob-viously hig her in the 51to 60-year-o ld group than in the 21to 30,31to 40an d41to 50-year-old groups(119±4),(76±5),(90±4)mm Hg ;(110±7),(68±5),(82±4)mm Hg;(112±8),(70±4) ,(84±4)mm Hg;(112±5),(71±6),(86±4)mm Hg,P<0.0 1.③The diurnal mean systolic pressure,mean diastolic pressure a nd mean ar terial pressure were obvio usly higher in the 51to 60-year-old group than in the 21to 30,31to 40and 41to50-year-old groups(P<0.01). ④The nocturnal mean diastolic
