小剂量阿司匹林对诱导排卵增殖期子宫内膜及血流的影响

摘　　要：目的探讨小剂量阿司匹林是否能改善氯米芬诱导排卵时增殖期子宫内膜的发育和子宫血流灌注及其可能的机制。方法将30例不明原因不育妇女随机分为两组,试验组(氯米芬+阿司匹林)、对照组(氯米芬)各15例。观察HCG日子宫内膜厚度、子宫动脉搏动指数,并取内膜检测雌激素受体。结果试验组内膜厚度与对照组间差异有显著性意义(P<0.05);试验组子宫动脉搏动指数与对照组间差异有显著性意义(P<0.05),且试验组子宫内膜雌激素受体含量与对照组间差异有显著性意义(P<0.05)。结论小剂量阿司匹林能改善氯米芬诱导排卵时的子宫血流灌注,增加内膜雌激素受体表达,从而改善内膜发育不良的目的。Objective To assess the effect of clomiphene citrate(CC) on the development of the endometrium and uterine blood flow by the addition of low-dose aspirin and to explore the possible mechanics.Methods 30 women with unexplained infertility were divided into 2 groups randomly.15 cases were in each group.In the experimental group,women were treated with low-dose aspirin during ovulation induction using CC;in the control group,women were treated with the same ovulation induction regimen except aspirin.Endometrial thickness,pulsatility index (PI) of uterine artery were determined by utrasonography at the day of HCG injection.Endometrial biopsy was taken and SP immunohistochemical techniques for the ER was performed on it.Results There were statistically significant differences in endometrial thickness and the expression of ER was distinctly increased in the experimental group compared with the control group(P<0 05).PI values of experimental group were statistically significant lower than that in the control group(P<0 05).Conclusion Low-dose aspirin can improve uterine perfusion and estrogen receptor of endometrium and reverse the deleterious effect of CC on endometrial development.
