摘 要:目的为设计一种新术式(带血管桥的多根肋骨瓣)修复手掌毁损伤提供解剖学依据。方法在30侧经动脉灌注红色乳胶成人背部标本上,观察第7、8、9肋肋角外侧8cm段的形态结构及肋骨间血管、神经在该段的走行、分支、分布和吻合;在2侧尸体标本上模拟手术。结果该段的肋骨高[(1.4±0.2)cm,x±s,下同],肋间隙宽(1.0±0.3)cm,肋间动脉、静脉于该段的起始外径分别为(1.4±0.2)mm、(2.1±0.4)mm,终末支外径分别为(1.0±0.2)mm、(1.3±0.3)mm。结论设计带血管桥的多根肋骨瓣修复手掌毁损伤在解剖学上具有可行性。Objective To provide a new therapeutic method for severe palm injury. Methods The shape of the rib segment which was 8cm long starting at the costal cormu in the 7th 8th 9th rib,and the course,branch,distribution,anastomosis of the intercostal vessel by the rib segment were observed in 30 adult back specimens injected with red latex in artery;simulated operations were performed in two specimens. Results The height and thickness of the rib segment was(1.4±0.2)cm and (0.6±0.1)cm.The intercostal vessels were in the costal grooves of the rib segment;at the medial end of the segment,the outer diameter of the intercostal artery,vein were respectively(1.4±0.2)mm, (2.1±0.4)mm;at the lateral end of the segment,those were(1.0±0.2)mm, (1.3±0.3)mm.Conclusion It is feasible on anatomic to design multi-rib flap with vessel bridges to repair severe palm injury.
