摘  要:目的 探讨应用G CSF动员外周血干细胞时,异体 自体造血干细胞移植供 受者的骨髓及外周血CD3 4+ 细胞和T细胞亚群的动态变化特点及最佳外周血干细胞采集时间。方法 对12例健康供者和16例化疗7d后的恶性血液病患者,以G CSF 15 0 μg 12h ,皮下注射,连用5d。应用流式细胞仪测定骨髓及外周血CD3 4+ 细胞和T细胞亚群。结果 ①应用G CSF前,两者骨髓CD3 4+ 细胞有显著差异(P <0 0 1)。应用48h后,两者的骨髓CD3 4+ 细胞均较前有明显增加(P <0 0 1) ,两者之间仍存在明显差异(P <0 0 1)。两者的骨髓CD3 4+ 细胞高峰值出现在96h后,较应用前差异显著(P <0 0 1) ,两者之间无差异(P >0 0 5 )。②应用G CSF前,两者的外周血CD3 4+ 细胞无明显差异(P >0 0 5 )。应用72h后,两者外周血中CD3 4+ 细胞均较前有明显增加(P <0 0 1) ,两者之间无明显差异(P >0 0 5 )。96h后,两者的CD3 4+ 细胞达高峰,较应用前差异显著(P <0 0 1)。两者之间无明显差异(P >0 0 5 )。③应用G CSF对两者的T淋巴细胞亚群无明显影响。结论 应用G CSF动员外周血干细胞96h后,异体 自体造血干细胞移植供 受者的骨髓及外周血CD3 4+ 细胞均达到高峰,可适时采集外周血干细胞。Objective To explore the dynamic changes of CD34 + cells and T lymphocyte subsets and best time of harvesting peripheral blood stem cell when G-CSF was used for peripheral blood stem cell mobilization in donors and patients. Methods A total of 12 donors and 16 patients who received chemotherapy for 7 d were injected G-CSF of 300 μg/d to mobilize peripheral blood stem cells for 5 d and flow cytometry were used to detect the changes of CD34 + cells and T lymphocyte subsets everyday for 5 d. Results ① Before G-CSF treatment, there were obvious differences in bone marrow CD34 + cells between patients and donors (P<0.01). After 48-hour treatment, the amounts of CD34 + cells of both groups were obviously increased than before treatment (P<0.01). The highest amounts were reached after 96-hour treatment with significant difference between two groups (P<0.01). ② Before G-CSF treatment, there was no obvious difference between patients and donors in peripheral blood CD34 + cells. After 72-hour treatment, there was obvious increase in peripheral blood CD34 + cells in patients and donors (P<0.01), but there was no obvious difference between each other. After 96 h, the highest values were reached, which were higher than that before G-CSF treatment (P<0.01), but no difference between two groups. ③ There were no obvious changes in the donor's and patient's T lymphocyte subsets after using G-CSF for some days. Conclusion After mobilization for 96 h, the highest amounts of CD34 + cells in bone marrow or peripheral blood of patients and donors were reached and peripheral blood stem cells can be harvested.
