前路减压、植骨加Kaneda内固定治疗胸腰椎严重爆裂性骨折

摘　　要：目的探讨前路减压、植骨加Kaneda内固定术治疗胸腰椎严重爆裂性骨折的临床疗效。方法回顾分析1999年-2004年11例前路减压、植骨加内固定术治疗的腰椎严重爆裂性骨折患者的术后疗效和随访情况。结果所有患者术中及术后均未出现严重并发症。术后影像学检查除两例有轻度脊柱侧弯外,其余均见脊柱序列和椎管矢径恢复正常。11例患者中9例获3-20个月的随访(平均随访5.7个月),平均融合时间3.6个月。患者术后神经功能恢复较好,随访时后凸矫正度无明显丢失。结论腰椎严重爆裂性骨折采用前路减压,植骨加Kanada内固定术具有良好的减压复位及固定效果。Objective To investigate the applied value of anterior decompression, grafting and internal fixation with Kaneda device in the treatment of severe thoraco- lumber spinal burst fracture. Methods To respectively analyze curative effect and the state of follow - up of 11 patients with severe thoracic and lumbar spinal burst fracture who were performed with anterior decompression, au-tograft and internal fixation with Kaneda device . Results In all patients severe complications had not appeared in or after operation. 9 of 11 cases were followed up 3 - 20 months (average of 5.7 months ) . The recover of neural function of all patients were very good and the kyphosis had not lost obviously. Conclusion Anterior decompression, grafting and internal fixation with Kaneda device has effective decompression, well diaplasis and favorable stabilization.
