摘  要:目的调查长期服用氯氮平对精神分裂症患者糖代谢的影响。方法根据空腹静脉血和餐后2小时静脉血的血糖水平,对长期服用氯氮平的男性精神分裂症患者进行糖尿病及与糖尿病相关状态的诊断。结果①在90例患者中有54例(60%)糖代谢正常,9例(10%)为糖尿病,9例(10%)糖耐量减退(IGT),18例(20%)空腹血糖损害(IFG)。②将患者按体重指数(BMI)、氯氮平剂量、病程和年龄的不同分成3个组,3组之间糖代谢异常的发生率无明显差异。③不同糖代谢组间的BMI的差异有显著性(P<0.05),IGT组与正常组间的BMI差异有显著性(P<0.01)。④在IFG组,餐后血糖(2hPG)和甘油三脂与BMI呈显著性正相关(P<0.01),Logistic回归分析发现病程(P<0.05)、BMI(P<0.05)对糖代谢异常的发生有显著性意义上的作用。结论长期服用氯氮平对精神分裂症患者糖代谢有影响,BMI和病程是发生糖代谢异常的危险因素。AIM To study the effects of clozapine on glucose metabolism in chronic schizophrenic patients.METHOD According to the blood glucose level of fasting plasma and 2 hours plasma,We diagnosised of diabetes mellitus,impaired fasting glucose (IFG) and impaired glucose tolerance (IGT) which are associated with diabetes mellitus.RESULTS (1) 54 cases (60%) were normal in glucose metabolism,9 cases (10%) were diabetes mellitus,9 cases (10%) were IGT and18 cases (20%) were IFG,.(2) The rates of glucose metabolic conditions were no different among the three different subgroups when the patients were divided into three subgroups based on the difference of BMI,dose of clozapine,ill course and ages.(3) The differences of BMI were significant among the different groups of glucose metabolic conditions (P=0.005),There was a significant difference between IGT group and the normal glucose metabolism group.(4) In IFG group,2hPG and triglyceride were significantly positive correlation with BMI (P=0.001、0.008 respectively).By the Logistic regression,it was found that the ill course and BMI were the significant factors in the development of glucose metabolic disorder.CONCLUSIONS There were effects of clozapine on glucose metabolism in chronic schizophrenic patients.Ill course and BMI were the significant risk factors in the development of glucose metabolic disorder.
