巯基蛋白酶抑制肽C检测肾小球滤过功能的临床研究

摘  要:目的用血清巯基蛋白酶抑制肽(CcystatinC)检查肾小球滤过功能,同时与血清肌酐作比较。方法总测试人数155例,其中儿童组89例,年龄从出生到13岁,均为住院恢复期病人,无肝、肾疾患;成人组66例,其中健康志愿者20例(男12例,年龄36.3±9.5岁,女8例,年龄34.3±10.6岁),患者46例(男26例,女20例),均为肾内科住院病例。用全自动生化分析仪,颗粒增强透射免疫比浊法测定血清cystatinC,肌氨酸氧化酶-PAP法测定血清肌酐。结果儿童组1~13岁,血清cystatinC的浓度稳定于1.26±0.33mg/L,而血清肌酐随年龄增长而增高,与血清cystatinC无相关性;成人组血清cystatinC的浓度稳定、与血清肌酐正相关。两组血清cystatinC均与年龄、身高、体重、性别无关。结论血清cystatinC浓度稳定,范围较窄,不受年龄、身高、体重、性别的影响,用以监测肾小球滤过功能优于血清肌酐。用cystatinC监测肾小球滤过功能有明显的优点,具有广泛的应用前景。Objective To study the use of serum cystatin C for estimation of GFR.Methods Serum cyslatin C were measured by particle-enhanced turbidimetric immunoassay (PETIA) and serum creatinine by the para-aminophenazone PAPin 155 subyects include children and adults.Results Serum cystatin c remained constant concentration range at 1.26±0.33 mg/L from1-13 years,serum creatinine concentration rose gradually with age after l year,serum cystatin C show no correlatiou to aged,gender,height,and weight.There were no corrdlation between cystatin Cand creatinine in children.correlation between Ser and Cer was better in adults.Conclusion Serum cystatin Cis independent of aged,gender,height and body composition.the measurement of serum cystatin Cis better than serum creatinine for monitirng glomerular filtration function
