肾脏疾病中的脂质异常及其危险因素分析

摘　　要：目的回顾性总结肾脏疾病患者脂质水平的变化,分析与其相关的危险因素。方法收集229例各种肾脏疾病住院患者的临床资料,记录一般情况、血脂水平、肾功能、尿蛋白水平等,应用SAS软件进行相关性分析。结果229例肾病患者中61.57%的甘油三酯(TG)、50.21%的总胆固醇(Tch)、27.95%的高密度脂蛋白(HDL)、34.50%的低密度脂蛋白(LDL)、21.40%的载脂蛋白A(apoA)、23.58%的载脂蛋白B(apoB)和41.92%的脂蛋白(α)[Lp(α)]高于正常范围的高限;4.37%的HDL低于正常范围低限。回归分析显示相关关系包括:TG与尿蛋白、体重、尿酸,LDL、Tch、apoB与尿蛋白,HDL与年龄、血清肌酐等,apoA与体重、血清白蛋白与球蛋白比,Lp(α)与血尿素氮、尿蛋白相关。结论在肾脏疾病人群中,异常脂血症表现多样且常见,主要相关危险因素是尿蛋白,也包括肾功能、血清白蛋白、血清白蛋白与球蛋白之比等。Objective To investigate dyslipidemias in patients with chronic kidney diseases(CKD) and analyze the related risk factors. Methods Two hundreds and twenty nine hospitalized patients with various renal diseases were enrolled. The data about general information, lipid metabolism, renal function and urine protein were collected. Relative analysis was performed by SAS software. Results Among 229 patients with renal diseases, there were 61.57% patients with high triglyceride (TG), 50.21% with high total cholesterol (Tch), 4.37% with low high-density lipoprotein (HDL), 27.95% with high HDL, 34.50% with high low-density lipoprotein (LDL), 21.40% with high apolipoprotein A (apoA), 23.58% with high apolipoprotein B (apoB), and 41.92% with high lipoprotein(α) \[Lp(α)\]. The results of regressive analysis showed that the risk factors of high TG, Tch and apoB were proteinuria, body weight and serum uric acid; low HDL was related with age, serum creatinine and serum albumin; high LDL related with proteinuria; high apoA related with body weight and serum albumin; high Lp(α) related with blood urea nitrogen and proteinuria. Conclusion Dyslipidemias are common in CDK patients. The main risk factors of dyslipidemias are proteinuria, low renal function and decreaeed serum albumin.
