摘　　要：目的为提高直接眉上提术的效果,延长有效时间,从理论与实际观察探讨眶周区域老年样改变形成的原因,采用相应的手术术式进行矫治。方法肿胀麻醉下,通过直接眉上提术眉区切口入路,在眼轮匝肌深面向外剥离,离断眶韧带并完全剥离开眼轮匝肌外侧眶部;分别暴露降眉肌和眉肌并离断;将眼轮匝肌上提分别固定于颞深筋膜及额骨骨膜上;最后缝合皮肤。结果本组患者共62例,术后眉外侧平均上提8mm,眶睑沟重新显现或加深,静态时眉间纹、鱼尾纹不可见。随访30例3～24个月,效果满意。结论利用直接眉上提术切口入路,减少上面部降肌的作用,相对增强升肌的作用,再辅以肌肉上提固定及皮肤拉紧而达到眶周区域年轻化目的,且手术入路瘢痕不明显。效果确实、可靠、安全、持久。Objective To improve the effect of eyebrow suspension and prolong the effective time, we study the cause of periorbital senile change through theory and practice. Methods In turgescent anesthesia,stripping to orbital region under musculus orbicularis oculi at the approach of eyebrow area.The orbital ligament is cut, and the superciliary corrugator muscle and depressor muscle are exposured and cut. The musculus orbicularis oculi is fixed to deep temporal fascia and frontal periosteum. At last the skin is sutured. Results In 62 cases, lateral eyebrow was average raised to 8?mm. Orbital-blepharal groove appeared again or was deeper. Glabellar wrinkle and canthal wrinkle disappeared at static state, and decreased at dynamic stste. 30 cases were followed-up for 3-24 months. The results were satisfied. Conclusion By the approach of direct eyebrow suspension, the use of depressor muscle is decreased, the use of elevator muscle is enhanced. Periorbital area is young with muscle suspension and skin lift.The scar isn′t obvious. The effect is reliable, safe and lasting.
