某钨矿矽肺并发肺结核的调查分析

摘　　要：目的探讨矽肺并发肺结核的发生和发展的情况。方法对某钨矿440例矽肺并发肺结核患者(矽结组)进行回顾性调查,并以该矿329例单纯矽肺患者作为对照组。结果在该矿769矽肺中,Ⅰ、Ⅱ、Ⅲ期矽矽肺并发肺结核率分别为62.53%、34.48%和93.33%,有显著性差异(P<0.01=。矽结组Ⅰ、Ⅱ期患者发病年龄均小于对照组,但无统计差异(P>0.05);凿岩工的患病率明显高于对照组(P<0.05),其他工种两组无差异(P>0.05);Ⅰ期患者的接尘工龄显著短于对照组(P<0.01)。矽肺并发肺结核率随年代的增加随之降低。矽结组各晋期的时间无显著性差异(P>0.05)。矽结组病死率显著高于对照组(P<0.01),并随着矽肺期的增加而随之增高(P<0.01)。结论矽肺并发肺结核患者多发于凿岩工、接尘工龄较短、病死率较高、并发肺结核率随年代增加而降低。该矿预防和控制并发肺结核工作是有成效的。Objective To explore the occurrence and the development of the silicosis patients complicated with pulmonary tuberculosis. Methods A retrospective study was conducted in 440 wolfram mine silicosis patients complicated with pulmonary tuberculosis(SCPT group) and 339 simple silicosis in the same wolfram mine as control group. Results The incidence of complicated with pulmonary tuberculosis among Ⅰstage, Ⅱstage and Ⅲ stage silicosis were 62.53%, 34.48% and 93.33% respectively(p<0.01).There were no statistical differences between the age of pa- tients suffering from silicosis in Ⅰstage and Ⅱstage in SCPT group and control group(p<0.05) .The incidence of rock drilling workers was higher than that in the control group (p<0.01),and there were no differences in the other types of work(p>0.05). The contact period of the Ⅰstage silicosis was significantly shorter than the control group (p<0.01). The incidence of silicosis patients complicated with pulmonary tuberculosis became lower as time passed. The stage promoting interval of SCPT group has no statistical differences (p>0.05). The case fatality rate of the SCPT group was significantly higher than the control group(p<0.01),and became higher accompanying with the silicosis(p<0.01). Conclusion The silicosis patients complicated with pulmonary tuberculosis were happened generally in rock drilling workers and shorter period of contacting dust workers. The mortality rate was higher. The incidence of silicosis patients complicated with pulmonary tuberculosis became lower as time passed. The measures of control and prevention of silicosis complicated with pulmonary tuberculosis were effective.
