前列地尔脂微球注射液治疗糖尿病神经损害及皮肤溃疡52例

摘　　要：目的:观察前列地尔脂微球注射液(Lipo PGE_1,商品名凯时)与前列地尔粉针剂(PGE_1)在糖尿病神经损害及皮肤溃疡治疗中的临床疗效和安全性。方法:糖尿病伴有神经损害及皮肤溃患者102例,其中使用LipoPGE_152例,PGE_150例,使用方法:Lipo PGE_1 10μg静脉推入,qd;PGE_1 100ug加入500mL生理氯化钠溶液静脉点滴,qd,均用药2-4周。结果:Lipo PGE_1组改善静息痛、感觉异常及下肢溃疡的有效率(分别为60.9%,58.3%,65.0%)均显著高于PGE_1组(分别为42.8%,45.0%,44.5%)(均P<0.01),经皮氧分压测定末梢循环血量、血氧分压明显增高,正中神经和腓神经的MCV和SCV明显改善,但不良反应发生率和程度明显小于PGE_1组。结论:Lipo PGE_1治疗糖尿病神经损害及糖尿病皮肤溃疡疗效肯定,不良反应小。Objective: To evaluate the efficacy and safety of lipo prostaglandin E_1 (lipo PGE_1) and prostaglandin E_1(PGE_1) in diabetic patients with peripheral neuropathy and skin ulcer. Methods: One hundred and two diabetic patients with peripheral neuropathy and skin ulcer were randomly i. v. received with either lipo PGE_1 (10μg, n=52) once daily or PGE_1 (100μg, n=50) once daily for 2～4 weeks. Results: The lipo PGE_2-treated patients experienced significant improvements in spontaneous pain, numbness and skin ulcer compared with the PGE_1-treated patients, 60.9% vs 42.8% ,58.3% vs 45.0 % and 65.0 % vs 44.5 %, respectively ( P<0.01). A transcutaneous oxygen tension (TcPO_2) study had shown that lipo PGE_1 significantly improved the peripheral circulation, degree of blood oxygen saturation and MCV and SCV of median nerves and peroneal nerves. The patients who were treated with the lipo PGE_1 experienced less incidence of adverse events compared with those in the PEG_1 group. Conclusion: The lipo PGE_1 showed the efficacy and safety in the treatment of diabetic patients with peripheral neuropathy and skin ulcer.
