前列地尔脂微球载体制剂治疗肺心病心力衰竭合并心绞痛患者的疗效

摘　　要：目的:评价常规治疗肺心病心力衰竭合并心绞痛加用前列地尔脂微球载体制剂(Lipo-PGE_1)的临床疗效。方法:将56例肺心病心力衰竭合并心绞痛患者随机分成两组,试验组32例,在常规治疗基础上加用Lipo-PGE_1 5μg·d^(-1),疗程平均15d;对照组24例,为常规治疗。观察各组治疗前后及组间临床表现、心功能分级、心电图、超声心动图、全血黏度、红细胞压积、血沉和血气分析的变化。结果:试验组显效19例(59.4%),有效12例(37.5%),总有效率96.9%,;对照组显效5例(20.8%),有效11例(45.8%),总有效率66.7%,两组总有效率比较有显著性差异(P<0.05)。试验组治疗后全血黏度、红细胞压积、血沉、心脏射血分数和PaO_2的改善显著大于对照组(P<0.05)。结论:加用Lipo-PGE_1治疗肺心病心力衰竭合并心绞痛,比单用常规治疗疗效更明显,使用方便安全。Objective: To evaluate the efficacy of lipo prostaglandin E_1 (Lipo PGE_1) in the treatment of cor pulmonale with heart failure and angina pectoris. Methods: Fifty-six patients with cor pulmonale with heart failure and angina pectoris were randomized to one of the treatment groups: active group (n=32, conventional therapy plus Lipo PGE_1 at 5μg·d^(-1), iv drip) and control group (n = 24, conventional therapy only) for 24 days. The evaluation of efficacy was conducted based on the variables of clinical manifestation (NYHA functional class), cardiogram, echocardiography, hemorheology, hematocrit, erythrocyte sedimentation rate and blood gas analysis before and after the treatment. Results: The overall response rate in the active group was 96.9 percent compared with 66.7 percent in the control group (P<0.05). 59.4 percent of patients (19 out of 32) in the active group experienced a complete response to the therapy versus 20.1 percent (5 out of 24) in the control group. 12 patients in the active group had improvements (37.5 % ) versus 11 patients in the control group (45.8 %). Conclusion: The adjunctive regimen of Lipo PGE1 with conventional therapy offers a more superior option than the conventional therapy alone in the treatment of eor pulmonale with heart failure and angina pectoris.
