摘  要:目的分析全胃肠外营养中心静脉导管感染菌群的耐药情况,指导临床用药。方法回顾性调查1998年1月至2002年12月在我院发生全胃肠外营养中心静脉导管感染57例病原菌及药敏试验结果。结果感染菌群中革兰阳性菌占43.9%,革兰阴性菌占49.1%,真菌占7.0%。多数细菌对不同抗菌药物有不同程度的耐药,革兰阳性细菌对万古霉素的敏感率是100%,革兰阴性细菌对头孢他啶和头孢曲松的敏感率分别为78.6%和64.3%。结论临床医师及早做细菌培养,根据药敏试验选择应用抗菌药物治疗导管感染,可提高治愈率。Objective To study antibiotic resistance of pathogens isolated from patients with catheter-related infections receiving total parenteral nutrition. Methods Clinical data of 57 cases with catheter-related infections between January 1998 and December 2002 were analyzed retrospectively. Results Gram-positive cocci, gram-negative bacilli and fungi accounted for ~43.9 %, ~49.1 % and 7.0% respectively. Most bacterial isolates were resistant to various antibiotics to some extent. Gram-positive cocci were totally (100%) susceptible to vancomycin. About 78.6% and 64.3% of gram-negative bacilli isolates were susceptible to ceftazidime and ceftriaxone respectively. Conclusions Clinical specimens should be cultured for pathogenic agents as soon as possible. Antimicrobial therapy according to the results of susceptibility testing can improve the cure rate of catheter-related infections.
