带蒂皮瓣早期断蒂的动物实验与临床观察

摘　　要：功能位的局限性深度组织破坏，用含脂肪超厚带蒂皮瓣移植疗效甚佳。本组共27例（28个皮瓣），断蒂时间术后12～7天，平均8天，断蒂前不作常规的蒂部血运阻断训练，采用直接断蒂。皮瓣大小12×5～4×3cm2．平均9×5cm2，皮瓣全部成活。Distal pedicle flap has its advantages forrepairing functional site, local deep tissue damage,because of its simplicity in technique, good flexibilityand wearability and cheapness in cost. It is suitable tobe used in grass-root medical units. It was generallyrecognized that pedicle division should be done about3 weeks after operation and should spend a few daysto exercise blocking of blood circulation in the pediclebefore it is divided. This is a complex, time consuming and painful procedure. The author made anexperiment on rabbits, using 17 pieces of pedicle flapsand the pedicles were divided on the 5th, 6th, 7th,8th, 9th, 10th and 11th day after operationrespectively. Through flap temperaturedetermination, tissue sections and chromoscopicexamination, it was proved that it is feasible to dividethe pedicle on the 6th day after operation. Althoughthe skin structure and healing process differ verygreatly between man and rabbit, the result of theexperiment is still of value as a reference forclinicians.The author also performed clinical practice using28 pieces of pedicle flaps in 47 repairing sites. Theflaps were exposed to infrared radiation.Vasculodilator was used and the operation procedurewas improved. The average time for pedicle divisionwas shortened to be 8 days after operation. Thepedicle division was carried out in two groups. Thefirst group had 13 flaps. Blood supply to the pedicleswas cut down and observations were made to ensureeverything of the flaps was normal before the pedicleswere divided. The other group had 15 flaps. Thepedicles were divided directly without blocking bloodcirculation. Both groups gave the same results- Allthe 28 flaps survived.
