高频电刀对胆系手术切口愈合影响的临床观察  被引量:10

摘  要:目的 探讨高频电刀对手术切口愈合的影响。方法 随机将98例胆系手术患者分为电刀组(n=37)、电凝组(n=35 )、对照组(n=2 6 ) ,其中,电刀组剖腹时用高频电刀切割组织,并用电凝止血。电凝组用普通手术刀剖腹,用电凝止血。对照组用普通手术刀剖腹,用丝线结扎止血。观察3组患者术后切口愈合的情况。结果 电刀组有7例(18.92 % )、电凝组6例(17.14 % )、对照组1例(3.85 % )切口愈合延迟。经统计学处理,电刀组与对照组的差异有显著性(χ2 =4 .96 ,P<0 .0 5 ) ,电凝组与对照组的差异有显著性(χ2 =4 .0 6 ,P<0 .0 5 ) ,电刀组和电凝组的差异无显著性(χ2 =0 .0 6 6 ,P>0 .0 5 )。结论 高频电刀可使胆系手术切口愈合延迟率明显增加。因此,对胆系手术不宜使用电刀进腹,以免增加切口愈合不良的发生率。Objective To study the influence of high-frequency electric surgical knives on healing of biliary system incision.Methods 98 cases underwent biliary system operations were randomly allocated into three groups:electric knife(EK,37 cases),electro-coagulation(EC,35 cases)and control(26 cases).High-frequency electric surgical knives were used to cut abdominal tissues and electro-coagulation for hemostasis in EK group.Common lancets and electro-coagulation were applied to EC group.Common lancets and tieing silk suture were used in the control group.Results There were delayed wound healing in 7 cases (18.92%) in EK group,6 cases(17.14%)in EC group and 1 cases (3.85%)in the control group.There was a statistically significant difference between EK and the control group(χ 2=4.96,P<0.05),significant difference was between EC and the control group(χ 2=4.06,P<0.05),and difference was not significant between EK and EC group(χ 2=0.066,P>0.05).Conclusions High-frequency electric knives may remarkable retard the incision healing of biliary system.Electric knife should be avoided inside the abdomen in biliary system operation,and the occurrence rate of poor healing may be reduced herefrom.
