外伤性胸腰椎骨折的X线和CT诊断

摘　　要：目的 探讨Ｘ线和ＣＴ在外伤性胸腰椎骨折中的诊断价值。材料和方法５７例胸腰椎骨折均行Ｘ线和ＣＴ检查。结果５７例７４个椎体骨折中单椎体骨折４３例，多椎体骨折１４例。爆裂型骨折４５个椎体，压缩型骨折２１个椎体，骨折脱位型９个椎体。骨折发生于前柱１０个椎体，前中柱２２个椎体，前中后柱３９个椎体，中后柱２个椎体，前后柱１个椎体。结论Ｘ线平片对外伤性胸腰椎骨折有一定的价值，但ＣＴ在显示椎体三柱结构，骨折线，小关节脱位，椎管狭窄程度，脊柱稳定性，判断脊髓损伤等有明显优势。Objective To evaluate X - ray and CT diagnostic value in traumatic thoracolumbar fractures Materials and Methods All 57 cases of thoracolumbar fractures were examined by X ~ ray and CT. Results 43 cases are single vertebral fractures, 14 cases are multiple vertebral fractures. 45 vertebral bodies (VB) are burst fracture, 21VB compressive fracture, 9 VB fracture dislocation. Fractures oc- curred in the anterior column (10 VB), the anterior - middle column (22 VB), the anterior - middle - posterior column (39VB), the mid- dle - posterior column(2VB), the anterior - posterior(1 VB). Conclusion X - ray has some values in diagnosis of thoracolumbar fractures, but CT is superior to X - ray plainfilm in displaying the three columns structure of spine, fracture line, dislocation of zygapophysial Joints, narrowed canal and the stability of the vertebra and injury of spinal cord.
