摘 要:目的 探讨一氧化氮在硬化肝全胃肠外营养 (TPN)时进一步肝损害中的作用。 方法 2 6只肝硬化大鼠随机分成 4组 ,TPN组 7只 ,TPN +L Arg组 6只 ,TPN +L NAME组 6只 ,TPN +L Arg +L NAME组 7只。测定肝功能、肝甘油三酯 (TG)、胆固醇 (CHO)含量、血清NO浓度、肝脏NO含量、肝脏NOS比活性 ,观察肝脏 β NADPH黄递酶组化染色及肝脏病理组织学改变的情况。 结果 TPN组、TPN +L NAME组和TPN +L Arg +L NAME组各有一只大鼠因输液过快 ,肺水肿死亡 ,其余大鼠均成活。与TPN组比较 ,TPN +L Arg组血清NO浓度、肝脏NO含量明显升高 (P <0 .0 5 ) ,而血清AST明显降低 (P <0 .0 5 ) ,ALT降低但无统计学意义 ,肝TG含量明显降低 (P <0 .0 5 ) ,肝脂肪变性程度减轻。TPN +L NAME组血清NO浓度、肝脏NO含量明显降低 (P <0 .0 5 ) ,而血清AST、ALT明显升高 (P <0 .0 5 ) ,肝TG含量明显升高 (P <0 .0 5 )。各组血清ALB、A/G比值无明显差别。各组肝脏NOS比活性无明显差别 (P >0 .0 5 ) ,β NADPH黄递酶组化染色为 ++。 结论 一氧化氮在硬化肝全胃肠外营养时的进一步肝损害有保护作用。Objective To investigate the effects of nitric oxide (NO) on further hepatic injury of cirrhotic rats with total parenteral nutrition (TPN). Methods 26 cirrhotic rats are divided into four groups randomly:TPN group ( n =7),TPN+L-Arg group ( n =6),TPN+L-NAME group ( n =6) and TPN+L-Arg+L-NAME group ( n =7).Liver function,hepatic triglycerin (TG) and cholesterol (CHO),the level of serum and liver NO,liver nitric oxide synthase (NOS) activity were measured.β-NADPH diaphorase statining and histologic changes of the liver were observed. Results All animals survived but one rat died of pneumonedeman in TPN group,TPN+L-NAME group and TPN+L-Arg+L-NAME group respectively.Compared with TPN group,In the TPN+L-Arg group,the level of serum and liver NO were increased significantly ( P <0.05),however,serum AST was decreased significantly ( P <0.05) but ALT was not,liver TG was decreased significantly ( P <0.05) and the degree of fatty degeneration in the liver was lessened.In the TPN+L-NAME group,the level of serum and liver NO were decreased significantly ( P <0.05),however,serum AST and ALT were increased significantly ( P <0.05),the level of liver TG was increased significantly ( P <0.05).Serum ALB,A/G ration and liver NOS activity had not significantly changes among all groups.The degree of β-NADPH diaphorase staining was ++. Conclusions NO may play a protective role in further liver injury of cirrhotic rats with TPN.
