大鼠烫伤复合内毒素血症肾脏皮质微循环的变化及其机制的初步探讨

摘  要:利用大鼠完整的肾盂积水肾脏,观察了烫伤复合内毒素血症肾皮质微循环的变化,并对其机制进行了初步探讨。观察中发现:大鼠肾皮质血管扩张、瘀血,血流速度减慢;红细胞聚集,白细胞贴壁滚动;白细胞不仅粘附于小静脉壁,而且还粘附于小动脉壁;其微循环障碍重于单纯烫伤。丹参素能改善上述微循环障碍。烫伤复合内毒素血症血清PLA_2水平显著升高,为87.3±27.1U/ml(对照组:32.5±16.8U/ml,烫伤组:61.7±16.6U/ml,丹参素组:38.8±15.3U/ml,P<0.05)。上述结果表明:烫伤复合内毒素血症大鼠肾皮质微循环严重障碍,可能与血清PLA_2的水平有一定的关系,丹参素能改善上述肾皮质微循环障碍。The changes of renal cortex microcirculation in rats with burns compained with endotoxemia were observed by making use of the intact hydronephrotic kidney and their related mechanisms were studied. The venules dilated and congested, while erythrocytes aggregated and leukocytes rolled along the vessel wall. The leukocyte adhesion occurred in not only the venules but also the arterioles. The extent of above damage was more severe than that in burns. The concentration of phospholipase A_2(PLA_2) in serum increased markedly (Burns+Endotoxin:87. 3±27. 1U/ml; Control: 32. 5±16.8U/ml; Burns: 61. 7±16. 6U/ml, P<0. 05). DS-182 was able to reduce the count of adhesive leukocytes and decrease the level of PLA_2 in serum (38. 8±15. 3U/ml, P<0. 05). The results suggested that renal cortex microcirculation disturbance was more severe and was perhaps related to the level of PLA_2 in serum. DS-182 could improve the renal cortex microcirculation.
