糖耐量正常的糖尿病高危者的胰岛功能初探

摘　　要：目的观察糖耐量正常(NGT)的糖尿病高危因素者的胰岛功能状况。方法选取糖耐量正常(NGT)的高血压、高血脂、肥胖者21例为高危组,糖耐量低减者(IGT)20例为IGT组及同期经体检合格的健康人16例为健康组。行75g葡萄糖耐量试验(OGTT)测定空腹血糖(FPG)、空腹胰岛素(FINS)、空腹C肽(FCP),饮糖水后2h血糖(PPG)、胰岛素(PINS)及C肽(PCP),按照HOMA法计算胰岛素敏感指数(IAI)、胰岛素抵抗指数(IR)、胰岛素分泌指数(HBCI)和空腹β细胞功能指数(FBCI),3组进行统计学比较。结果高危组的FPG、IR、HBCI较健康对照组高,差异有显著性,但两组的IAI、FBCI无差异。高危组的FPG、PINS、PCP低于IGT组,差异有显著性,但两组的IAI、IR、HBCI、FBCI差异无显著性。结论糖耐量正常的高血压、高血脂、肥胖者的胰岛素抵抗和胰岛分泌功能已有部分改变,有别于健康人而与IGT者大致相似。Objective To observe the islet β-cell function of the patients of normal glucose tolerance (NGT) with hypertension, hyperlipemia and obesity which are the high risks of diabetes. Methods 21 cases of patients of NGT with hypertension, hyperlipemia and obesity(high risk group), 20 cases of IGT(IGT group) and 16 cases of healthy volunteers(healthy group),all of whom are the railway workers seeking medical treatment in out-patient department, were observed. The fasting plasma glucose (FPG),insulin(FINS), C-peptide(FCP) and pasting plasma glucose(PPG),insulin (PINS), C-peptide(PCP) were determined with OGTT. On the basis of FPG and FINS, insulin secretion function (including Homa β-cell function index-HBCI and fasting β-cell function index-FBCI) and insulin resistance level (including Homa insulin resistance index-IR and insulin action index-IAI) were calculated. The data from three groups were analyzed statistically. Results FPG, IR, HBCI of the high risk group are more than those in the healthy group with significant difference, however, IAI,FBCI between the two groups showed no significant difference. FPG, PINS and PCP of high risk group are less than those of IGT group with significant difference, but IAI, IR, HBCI, FBCI between the two groups showed no significant difference. Conclusion the insulin resistance and islet β-cell function of the NGT patients in high risk of diabetes such as hypertension, hyperlipemia, and obesity have varied comparing with healthy group, and close to the level of IGT group.
