大面积皮肤撕脱伤诊治的回顾与探讨(附86例分析)

摘　　要：目的回顾大面积皮肤撕脱伤的临床资料,探讨行之有效的诊治措施。方法对1986年至2004年2月收治的86例大面积皮肤撕脱伤进行回顾性分析总结。结果86例病例主要由于车轮或机器传动带等产生的外力作用所致,在治疗上根据病情采取原位缝合,撕脱组织切除植皮及吻合血管的撕脱皮瓣回植等针对性治疗。结论大面积皮肤撕脱伤的治疗方案应根据伤情特点来选择,早期血液循环的判断和治疗方案的确定是临床预后的关键。Objective To review the material on the large area of clinical scalp avulsion injury and explore the effective management.Methods This article analyses 86 cases of large scalp avulsion injury treated by the authors from 1986 to Feburary 2004.Results The victims were mainly owing the force caught up in moving wheel or driving belt of a machine.In terms of treatment,we choose to sew up the original injury,to graft the split-skin which had been cut off or to anastomose the vessels of the split-skin,the replant based on the real situation of the victim.Conclusion The treatment scheme on large split-skin should be chosen based on the charcter of real cases.And it is also the key interfering with the result in trials for us to confirm early blood-circle and the treatment scheme to be used.
