自体血库、自体血液回输在膝关节置换术中的应用

摘　　要：目的:探讨自体血回输技术在膝关节表面置换术后应用及自体血库在膝关节置换术中的临床应用效果。方法:收集2003年1月6日～2004年12月12日,我院治疗膝关节置换36例病人、46个膝关节行表面置换手术。其中30例行40个膝关节置换术后采用自体血回输技术;10例病人在术前收集自体血液保存再在术中回输,并收集术后出血及时输注给病人,减少使用同种异体血,统计输血量及术前、术后2天、术后7天血常规,并与未行自体血回输技术的同种病人的数据对照。结果:20例单膝置换病人术后回输红细胞150～600ml,平均450ml;10例双膝置换病人术后回输红细胞450～1650ml,平均1060ml,单个膝关节置换术后平均输注同种异体血480ml。合并使用自体血库者术后回输红细胞200～500ml,平均360ml,术后输入同种异体血平均280ml(0～500ml),较未行自体血回输技术的同种手术病人输血量明显减少。结论:在行膝关节置换中,采用自体血库及血液回输技术,提高手术安全性,减少围手术期同种异体血的输血量,降低病人医疗费用。Objective:To explore the effect of the postoperative autologous transfusion salvage and preoperative autologous reserve transfusion on the total knee arthroplasty operation.Methods:46knees of36patients underwent the total knee arthroplasty operation.Autologous transfusion salvage were used after the operation of40knees30patients and autologous reserve transfusion were used in the operation of10patients.We accounted the volumes of homologous blood transfusion and blood routine test in preoperative、2days after operation、7days after operation of them,and then compared with that of the patients who underwent the same operation without using the salvage technique.Results:In20patients who underwent single total knee arthroplasty operation,average volume was about450ml(150～600ml).In10patients who had underwent double total knee arthroplasty operation the homologous blood.All of the patients need homologous blood transfuslon about480ml,but the volume of homologous transfusion was much less than that of the patients who didn`t accept the blood cell salvage after operation.Conclusion:The postoperative autolˉogous transfusion salvage and operative autotransfusion reserve in the total knee arthroplasty can improve the security of operation,decrease the blood volume of the homologous transfusion in the term between the operation,reduce the medical cost.
