重组合异种骨对兔股骨头缺血坏死模型实验修复过程的影响

摘  要:目的 本文旨在探讨含骨形态发生蛋白(BMP)的重组合异种骨是否对液氮冷冻兔股骨头缺血坏死模型有促进修复作用。方法 本实验采用新西兰大白兔24只,雌雄各半,体重2kg。用液氮冷冻法造成左侧股骨头坏死,并且在同侧股骨颈后方头颈交界处钻直径2mm孔至软骨下,随机分成实验组和对照组各12只,实验组术中植入重组合异种骨(RBX)30mg/只(含BMP1.5mg),对照组术中植入同体积的牛松质骨,于术后2、4、8周分别处死实验组和对照组各4只动物,观察有关指标。结果(1)X线检查结合计算机相对骨密度分析:2周时两组股骨头骨密度均未见增高,4、8周时均增高,但手术侧实验组股骨头相对骨密度显著高于对照组(P<0.05)。(2)单光子骨密度(BMD)测定:4周以后手术侧实验组股骨头骨密度显著高于对照组(P<0.05)。(3)组织学观察:4周时实验组手术侧股骨头内纤维组织增生,有大量的软骨形成和成骨细胞增生明显,新骨形成多于对照组,8周时实验组手术侧股骨头内坏死骨组织修复大部分已完成,表面软骨再生。而对照组坏死骨修复仍在大部分区域内进行,植骨仍有部分未被吸收。结论 重组合异种骨对液氮冷冻兔股骨头坏死实验修复过程有明显的促进作用。Objective To study the possible therapeutic effect of rhBMP on the repairing process of necrosis of the femoral head induced by liquid nitrogen in rabbits. Methods Osteonecrosis in the left femoral head was induced by freezing with liquid nitrogen in 24 Newland rabbits which were allocated randomly into the test and control groups, rhBMP was implanted in the test group from admist the operation,those receiving only the bovine cancellous bone served as the control.Four rabbits in the test group and control groups each were sacrificed successively at 2、4、8weeks after operation. Results Bone density study revealed that the relative bone density was significantly increased in the group compared with that of the control group after four weeks by computer assisted X-ray film analysis. there was more new bone formation in the test group than that in the control group. Conclusion Bone morphogenetic protein had apparently a promotion effect on the repairing process of liquid nitrogen induced necrosis of the femoral head in rabbits.
