曲马多雾化吸入应用于咽部术后镇痛的临床研究

摘　　要：[目的]探讨超声雾化吸入曲马多在咽部手术后镇痛的效果,寻找曲马多新的用药途径。[方法]将84例气管内全身麻醉行咽部手术的病人随机分为试验组和对照组,试验组雾化吸入曲马多10 0mg加生理盐水10mL ,对照组为生理盐水10mL。采用McGill疼痛评分法评估吸入前后的疼痛程度,并监测呼吸、心率、平均动脉压、血氧饱和度,记录首次进食时间。[结果]试验组雾化吸入曲马多15min后疼痛减轻,3 0min后达到稳定的镇痛效果,与对照组比较有统计学意义(P <0 .0 1) ;首次进食时间试验组为6.5 0h±1.44h ,对照组为9.3 2h±2 .42h ,两组比较有统计学意义(P <0 .0 1)。[结论]曲马多超声雾化吸入镇痛效果好,是一种安全、有效、无创、简便、新的给药途径。Objective:To explore the analgesic effect of ultrasonic nebulization of tramadol for postoperative patients after accepted pharyngeal surgery, to find a new way of administration of tramadol.Method:A total of 84 patients undergoing pharyngeal operation under general anesthesia via endotracheal medication were divided into test and control groups. Patients in test group inhaled 100 mg tramadol resolved with 10 mL normal salinevia ultrasonic nebulization, while those in control were given 0.9% sodium chloride ultrasonic nebulization only. The pain degrees of both group patients before and after the nebulization were evaluated with McGill pain scoring method, and indexes including respiration rate,heart rate,mean arterial pressure, blood saturation of oxygen of all patients were monitored and recorded.And the first time to start to eat of them was recorded as well.Conclusion:Patients of test group had the pain relieved 15 minutes after tramadol ultrasonic nebulization, and the analgesic effect could be stable half an hour after nebulization. Compared with patients of control group, there was a statistical difference between the two groups (P<0.01). The first time of starting to eat were 6.50 h±1.44 h after operation in test group and 9.32 h±2.42 h in control group respectively. There was also a statistical difference between them(P<0.01).Conclusion:Ultrasonic nebulization inhalation of Tramadol for patients has a good analgesic effect, which is a new, safe, effective, noninvasive and simple method of medication.
