经乳突后锁孔入路微血管减压手术治疗三叉神经痛

摘　　要：目的探讨三叉神经痛的手术方法及注意事项。方法本组14例三叉神经痛病人经乳突后锁孔入路微血管减压术,术中发现有明显血管压迫三叉神经,并与术前MRTA检查一致,在显微镜下显露出三叉神经根桥脑入脑段,注意处理好岩静脉,寻找责任血管后,用自体筋膜片将责任血管与神经根隔离。结果10例症状术后立刻消失,4例症状术后2天消失,1例3个月后疼痛复发。结论微血管减压术是治疗三叉神经痛的有效治疗方法。Objective To investigate the surgical methods and the matters needing of trigeminal nerve.Methods The operation was performed via retrosigmoid 'key hole' approach and microvascular decompression in 14 cases, It was obvious that the nerve was compressed the vascular during the operation, and it was the same as preoperative magnetic resonance tomographic angiography (MRTA), Its root entry zone (REZ) was exposed under the microscope, pay attention to manage the petrosal vein, It was found the offending vessel, a small piece self-fascia was inserted between the nerve and the vessel. Results 10 cases were free of the symptom immediately postoperative, 4 cases were free of the symptom in two days postoperatively and one case had recurrences of the symptom in three months postoperatively.Conclusion Microvascular decompression is a effective operation for trigeminal nerve[
