摘 要:目的探讨乙型肝炎病毒(HBV)血清学标志表现模式及转归。方法应用酶标记免疫吸附试验技术检测乙型肝炎病毒血清学标志物,共7754例乙肝患者。按文献报道[1]设定血清病毒学标记检测项目1~5项的排列顺序为(1)HBsAg(2)抗-HBs(3)HBeAg(4)抗-HBe(5)抗-HBc。以出现阳性序号为模式代码。结果本组乙肝病毒血清学标记模式分感染期模式组和恢复期模式组,两组共29种模式。其中感染期模式以135,145为主占43.39%,恢复期模式组以2,245,24,25,5为主占30.8%。本组对176例患者进行跟踪检测,时间为2~11年,结果显示,145及15模式,除少数为趋向恢复模式外,大多为长期携带模式。结论本组HBV血清免疫学标志模式较为复杂,除试验误差外,产生少见模式的主要原因,可能与低滴度HBsAg、抗-HBc、抗-HBs及乙肝抗原抗体免疫复合物血症有关。HBV血清学免疫标志模式可分为恢复期转换及HBV感染后长期携带的免疫状态固定模式。恢复期模式转换为e及s抗原转换、消失,继而产生高滴度抗-HBc,抗-HBe,抗-HBs。最终抗-HBc、抗-HBe相继消失从而表现为抗-HBs保护性抗体的模式。Objective Investigating the expression models and conversion results of HBV serological markers.Methods Applying the enzyme labeling immunoadsorption experment technique to decting HBV serological markers on all the 7754 cases of Hepatitis B. Guided by the literature arranging the order of sero-virological marker detecting one to five(1)HBsAg(2)anti-HBs,(3)HBeAg,(4)anti-HBe(5)anti-HBc. Generating the codes of models in order of appearance of positive sequence.Results HBV serological marker model were divided into the infective stage models and the convalescence modles in this group, 29 models altogether. Of which 135,145 were the main models and the occupied 43.39% in the infective stage models, 2,245,24,25,5 were the main models and occupied 30.8%. 176 patients were given following detection that lasted two to eleven years and the results showed the most of 145 and 15 models were long-term portable models but a few tended to restoration models.Conclusion HBV serological and immunological markers were a bit complicated in the group. The main reason of emerging seldom models may have something to do with low titer HBsAg, anti-HBs and hepatitis B immunocomplex hemato-index, besides experment errors. HBV serological and immunological markers were divided into the convalescence conversion models and the long-term portable immune state fixation models after infected by HBV. The convalescence model converted, e and s antigens converted, disappeared and prouduced high titer anti-HBc,anti-HBs. Finally anti-Hbe anti-HBe disappeared one by one and anti-HBs protective antibody modelappeared.[
