APACHEⅢ危重疾病评分系统在ICU中的应用价值

摘　　要：目的应用急性生理学和慢性健康状况评分(APACHEⅢ)评价重症监护病房(ICU)患者的病情和预后。方法选择我院2001年1月至2004年11月ICU中80例危重患者的资料用APACHEⅢ进行评分,分别计算存活组和死亡组的APACHEⅢ分值;每30分作为一个计分段,以60分作为界限分,每例患者分别评分并计算各组的病死率。结果死亡组APACHEⅢ分值明显高于存活组(P均<0.01);APACHEⅢ分值与病死率呈正相关,APACHEⅢ>60分患者实际病死率明显增高。结论APACHEⅢ评分能准确客观评价危重疾病的严重程度及预测预后。Objective To value the state of an illness and prognosis of patients in ICU with the acute physiological and chronic healthy evaluation Ⅲ(APACHE Ⅲ).Methods 80 cases suffered from severe diseases in our hospital from January 2001 to November 2004 were enrolled into the study,of which 42 cases in survival group and 38 cases in dead group.APACHE Ⅲ 《evaluating system of severe diseases》 was applied to evaluate the cases.And then the APACHE Ⅲ was calculated in survival group and dead group.Every 30 points was a score section,and 60 points was the limited.The mortality rate of each group was counted.Results The point of APACHE Ⅲ in dead group was significantly higher than that in survival group(P<0.01);There was positive correlation of the point of APACHE Ⅲ and the mortality rate;The mortality rate of patients whose APACHE Ⅲ>60 was increased significantly.Conclusion APACHE Ⅲ was applied to dynamic evaluate the patients in ICU in 2～7 days.It could value the severe degree and forecast prognosis exactly and objectively.
