摘  要:目的观察糖尿病肾病(diabeticnephropathy ,DN)患者尿中单核细胞趋化蛋白1(monocytechemoattractantprotein 1,MCP 1)的变化及雷公藤多甙(triptolide,TL)对其影响,探讨TL对DN的治疗作用。方法67例患者分为两组,治疗组(35例)采用TL和苯那普利联合治疗,对照组(32例)单用苯那普利治疗,疗程6个月,观察治疗前后尿MCP 1等指标的变化。结果DN患者尿MCP 1水平显著高于健康人(P <0. 0 1) ,治疗组治疗后尿MCP 1显著下降(P <0 . 0 5) ,与对照组比较差异有显著性(P <0 .0 5)。结论TL使尿中MCP -1的排出减少,抑制炎症反应改善肾功能。ObjectiveTo observe the change of urinary monocyte chemottractant protein-1(MCP-1) in patients with diabetic nephropathy (DN), and to explore the therapeutic effect and mechanism of triptolide (TL) in treating DN. MethodsThirty-five patients in the treated group were treated with TL plus benazepril and thirty two patients in the control group were treated with benazepril alone for six months. The change of urinary MCP-1 was measured before and after treatment. ResultsLevel of urinary MCP-1 in DN patients was significantly higher than that in healthy subjects (P<0.01), but it could be significantly decreased after TL treatment, showing significant difference as compared with that in the control group (P<0.05). ConclusionDetermination of urinary MCP-1 level is beneficial to know the degree of kidney inflammation in DN patients. TL can inhibit inflammatory reaction to decrease the level of urinary MCP-1, and thus improve the renal function.
