腹腔镜筋膜内子宫切除术59例分析

摘　　要：目的探讨腹腔镜筋膜内子宫切除术(CISH)的临床应用价值。方法回顾分析2000年3月～2003年12月我院59例CISH及50例经腹筋膜内子宫切除术的临床资料。比较两组手术时间、术中出血量、术后排气时间、术后病率、住院时间。结果腹腔镜组手术时间略高于开腹组,但两组比较无显著性差异(P>0.05),腹腔镜组术后排气时间与术后病率均低于开腹组(P<0.05)。腹腔镜组术中出血量、平均住院时间与开腹组比较均无显著性差异(P均>0.05)。使用血管闭合器组的手术时间、术中出血量均优于使用双极电凝器组(P均<0.01)。结论CISH手术创伤小,术后通气时间早。CISH手术时间及出血量与止血设备有直接的关系。值得临床推广应用。Objective To investigate the advantage and clinical value of Classical Intrafascial Semm Hysterectomy (CISH).Methods Fifty-nine cases of CISH and 50 cases of Laparotomy Intrafascial Hysterectomy performed in our hospital From March,2000 to December,2003 were retrospective analyzed for clinical data, operative time ,interoperation blood loss,postoperative exhaust time,rate of postoperative fever.Results In CISH group,the average operating time was slightly longer,the farting time and rate of postoperative fever were less than that of Laparotomy Intrafascial hysterectomy(P<(0.05)).The average bleed loss and in-hospital time had no significant deviations between two groups (P>0.05).By using Ligasure,the mean bleeding volume was reduced dramatically and the mean operating time was shortened(P<0.01).Conclusion The trauma of CISH is slighter and the exhaust time is shorter than that of Laparotomy Intrafascial hysterectomy.In the CISH group,there may be direct relation between the bleeding volume,operating time and hemostasis equipment.the CISH is worthy of being advocated.
