摘  要:目的研究糖尿病大鼠胃动力障碍发生的电生理机制及细胞病理学基础。方法同步记录正常对照组、中药组、链脲佐菌素所致糖尿病模型组清洁级雄性SD大鼠胃窦部的胃电活动,并应用NDP组织化学染色,用光镜及电镜观察各组大鼠胃动力基本功能单位,即壁内神经(ENS)鄄Caja1间质细胞(1CC)鄄平滑肌(SMC)的病理改变,并观察中药糖胃康制剂的防治作用。结果发现糖尿病组大鼠,胃电慢波频率及振幅明显下降;胃电节律失常;慢波频率变异系数(CV)及异常节律指数(ARl)显著增加;胃肌间神经丛的氮能神经分布及表达明显增强;胃动力基本功能单位有明显的病理改变,其平滑肌排列紊乱,并有较多胞质溶解性空泡、线粒体肿胀,核周细胞浆减少等。中药组大鼠ENS、ICC、SMC的病理改变明显轻于糖尿病组,胃电失常得到改善。结论糖尿病大鼠胃动力基本功能单位有明显的病理改变;胃电活动异常;胃壁NOS活性增强。中药使糖尿病大鼠胃电失常得到改善、胃壁NOS的含量降低、胃动力基本功能单位损害减轻。Objective To observe the electrophysiological mechanism and t he basic of cell pathology diabetic gastromotility disease (DGMD). Methods Male Sprague-Dawley rats of clean class(CL) were used for study. There are three gro ups of rats, diabetes mellitus(DM) was induced in these rats by streptozotocin a dministration in one group; the other two groups were Chinese traditional medici ne group and the control group. Record the changes of gastric myoelectrical in g astric antrum of all rats at the same time, then dyed by NDP by immunohistoehemi stry. Light and electron microscope were used to study the pathological changes of basic functional unit of gastric motility(BFUGM), i.e. enteric nervous system (ENS)-interstitial cells of Cajal (ICC)-smooth muscle (SMS) in rats of DM group and to observe the preventive effect of Chinese traditional medicine. Results ①Gastric myoelectrical measurement showed that the frequency and amplitude of s low wave were reduced significantly in DM group, the electric rhythm of electrog estric activity is abnormal.②Slow frequency confficient (CV) and abnormal rhyth m index (ARI) increased significantly. The distribution and expression of interm uscalar nitrogen nervous plexus increased significantly. ③There were marked pat hological changes of BFUMG in DM group, including disorganized arrangements, cyt oplasmic dissolving vacuoles and swelling mitochondria in smooth muscle and dimi nished cytoplasmic around the nucleus. ④The pathological changes of BFUGM in ra ts of Chinese traditional medicine group were apparently less than that of DM gr oup and the abnormality of electrogastic activity was improved. Conclusion Ther e were marked pathological changes of BFUGM in diabetic rats,including the abnor mality of electrogastric activity and the improvement of gastric nitric oxide sy nthase (NOS). Chinese traditional medicine can improve the abnormality of electrogastri c ,release the content of gastric NOS and the harmless of BFUGM for diabetic rat s.
