摘  要:目的探讨免疫活性细胞在异种移植急性血管性排斥反应(AVR)中的意义。方法猪到猕猴腹腔内异位心脏移植,使用高纯度的眼镜蛇毒因子完全清除受体体内补体,建立AVR模型。常规病理检查,并采用免疫组化法检测移植心CD4+T细胞、CD8+T细胞和巨噬细胞浸润程度,检测细胞间黏附分子1(ICAM1)和肿瘤坏死因子α(TNFα)的表达水平。结果移植心内大量淋巴细胞浸润,其中50%为巨噬细胞,CD4+T细胞占15%,CD8+T细胞占25%;ICAM1和TNFα表达明显上调。结论细胞机制在异种移植AVR发生、发展过程中发挥重要作用。Objective To investigate the role of CD4^(+) or CD8^(+) T cells in acute vascular rejection (AVR) of pig-to-monkey cardiac grafts treated with a high cobra venom factor (Y-CVF).Methods Heterotopic xenogeneic heart transplantation in the abdominal cavity was performed using piglets as donors. Four monkeys served as recipients. Complete complement depletion was achieved in the recipients treated with repetitive doses of Y-CVF. The recipients were immunosuppressed with a combination of cyclosporine A, cyclophosphamide and steroids. When no myocardial contractions could be seen or felt, the xenografts were excised and examined histologically and immunohistochemically for intercellular adhesion molecule-1 (ICAM-1),tumor necrosis factor-alpha (TNF-α), CD4^(+), CD8^(+)T cells and macrophages.Results Four pig recipient hearts receiving Y-CVF therapy were survived 8, 10, 13, 13 days respectively. Some of CD4^(+)T cells (15 % of total leukocytes), CD8^(+)T cells (25 % of total leukocytes) and macrophages (50 % of total leukocytes) were found infiltrating the entire xenograft. Up-regulation of ICAM-1 on the graft endothelial cells and TNF-α in the interstitial were demonstrated in the rejected heart.Conclusion Lymphocyte infusion in xenografts may play an important role during the development of AVR. The immunosuppression agents are necessary to prolong xenografts survival.
