输卵管银夹绝育术效果与安全性的前瞻性流行病学调查

摘　　要：对河北（Ａ夹）、湖南（Ｂ夹）、山东（Ｃ夹）３种银夹采用多中心前瞻性研究，于１９９０年９月～１９９２年１２月在山西、河南、河北、湖南及山东５省共３９３２人进行了为期一年的观察，随访率９９．９５％。结果表明，术后一年绝育有效率Ｐｅａｒｌ指数与寿命表法两种方法，Ａ夹分别为９８．７４％、９８．７５％；８夹为９９．５９％、９９．５２％；Ｃ夹为９９．２０％、９９．１３％。经χ２检验差异无显著性。副反应：术后一年自述症状以腰痛多见，月经量、月经周期改变次之，三者发生率分别为４．４５％、２．８０％及２．１１％。盆腔检查所见：宫体炎、附件炎仅占０．６６％，未发现其他器质性病变；并对影响效果及副反应的因素进行了分析。研究结果表明，输卵管银夹绝育术效率高，副反应少，简便易行，有普遍推广价值。A multi-center prospective study of tubal sterilization with three types of silver clips fromHebei(type A),Hunan(type B)and Shandong(type C)was conducted in Shangxi,Henan,Hebei.Shandong and Hunan in 3932 subjects from September 1991 to December 1992.The follow-up rate was99.95%.The result demonstrated that the one year efficacv rates of sterilization by Pearl index and bylife-table were similar.They were 98,74%, 98.75% with type A clip,99.59%, 99.52% with type B clip.99.20%, 99.13% with type.C clip. Chi square analysis showed no significant difference. Side-effects suchas lumbar soreness,changes in amount and duration of menstrual flow。 The incidence of side-effects with3 types of silver clips was 4.45%,2.8%,2.11%, respectively Pelvic infection occurred in 0.66%. No othercomplications was found. It may be concluded that the tubal sterilization with silver clip has the good fea-tures of high efficacy,few side-effects, easy to perform and is worthy to adopt in family planning pro-gramme.
